Study Title: Medical-related stress in patients and parents attending Children’s Health Ireland

What is the purpose of this survey?
Attending hospital, having a physical health condition, or being injured can be stressful experiences for children and their parents. Most children and parents cope well with the stress of attending hospital but some experience difficulties afterwards. The purpose of this survey is to understand how many children and their parents experience stress-related mental health problems following their medical experiences. We also want to understand who is most at risk and why. This will enable us to understand the scope of the problem, better identify those in need of care, and provide excellent care for those that might need or want it.

Why am I being asked to take part?
You are being asked to take part because you are a parent of a child who has attended CHI more than one time.

Do I have to take part?
No, your participation is completely voluntary and you will not be asked for your name or for any other identifying information.

If I take part, what will I be asked to do?
Once you have read this information and you are happy to take part, you will then be asked to complete a questionnaire. If you decide not to take part, please be assured that this will have no impact on your child’s current or future care. If you choose to take part, the survey will take about 10-15 minutes to complete. You will not be able to exit and return to complete the survey once you have started. You can however stop your participation at any time you wish.

Will the data be confidential?
Yes, this survey is completely anonymous. Your answers will be treated confidentially and the information you provide will be anonymised within any publications. Please do not include your name, your child’s name or any other individuals in any responses. Your anonymous data will be held securely and can only be accessed by the investigators. All data will be deleted when the study and any subsequent publications or reports are completed, this usually takes about 5 years.
